****By donating, you have read and agree to the privacy statement and the eligibility rules to donate as stated below.
1. I am a Canadian Citizen or Permanent Resident of Canada.
2. Donation payments are from your personal account and not from a Corporation.
3. The funds are being donated by me and not being provided by others or other entities.
4. If a contribution is made on behalf of a spouse on any card transaction, the balance of the card must be paid out of joint bank account between you and your spouse.
